Live Life Well: Initiatives in NSW

Prevent Diabetes Live Life Well program
The Prevent Diabetes Live Life Well program is the largest community-based type 2 diabetes prevention intervention in NSW. It is based on international best available evidence.
The aim of the Program is to prevent or delay the onset of type 2 diabetes amongst participants aged 50 to 65 years who have been identified to be at high risk. Participants are screened and recruited via General Practitioners from three Divisions of General Practice, in collaboration with Sydney South West Area Health Service.
